چکیده

Introduction: Acute Lung Injury (ALI), and its more severe subset the Acute Respiratory Distress Syndrome (ARDS), are a major cause of mortality in the Intensive Care Unit. Mechanical ventilation, a supportive therapy necessary to sustain life in many cases, may contribute to and worsen ALI, termed Ventilator Induced Lung Injury (VILI). Fibroproliferation is an early response to lung injury . Indeed dysregulated repair resulting in pulmonary fibrosis may be at the heart of ventilator dependence in ARDS . Characterising the role of excessive lung stretch in contributing to aberrant repair mechanisms would contribute to our understanding of VILI and assist in developing strategies to hasten recovery from ARDS.

The fibroproliferative response in acute respiratory distress syndrome: mechanisms and clinical significance.

Acute respiratory distress syndrome (ARDS) continues to be a major healthcare problem, affecting >190,000 people in the USA annually, with a mortality of 27-45%, depending on the severity of the illness and comorbidities. Despite advances in clinical care, particularly lung protective strategies of mechanical ventilation, most survivors experience impaired health-related quality of life for yea...

Ventilator-associated lung injury.

Mechanical ventilation of disease-affected lungs, as well as being an inadequate mode of ventilation for initially healthy lungs, can cause significant changes in their structure and function. In order to differentiate these processes, two terms are used: ventilator-associated lung injury (VALI) and ventilator-induced lung injury (VILI). In both cases, lung injury primarily results from differe...

Critical role for CXCR2 and CXCR2 ligands during pathogenesis of ventilator-induced lung injury

Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are clinical syndromes (hypoxemia, decreased lung compliance, and bilateral pulmonary infiltrates) that have multifactorial etiologies either from direct or from indirect injury to the lung (1, 2). Histopathologically, there is an initial acute exudative phase involving an alveolar-capillary leak in conjunction with leukocyt...

Anti-CD11c antibody, Efalizumab attenuate ventilator-induced lung injury.

BACKGROUND The pathophysiology of ventilator-induced lung injury (VILI) involves multiple mechanisms including inflammation and inflammatory cells infiltration. The anti-CD11c monoclonal antibody, Efalizumab has been demonstrated to inhibit the T cell activation, migration and adhesion to keratinocytes. MATERIALS AND METHODS In this study, we induced lung injury with mechanical ventilation in...

Dos Santos, C. C. and A. S. Slutsky. Invited Review: Mechanisms of ventilator-induced lung injury: a perspective. J Appl Physiol 89: 1645–1655, 2000.—Despite advances in critical care, the mortality rate in patients with acute lung injury remains high. Furthermore, most patients who die do so from multisystem organ failure. It has been postulated that ventilator-induced lung injury plays a key ...
